Introduction I began practicing as a family physician in 1997 and spent the first 17 years in a 70+ physician primary care group that had a strong culture of quality and physician leadership. As a member of the Pisacano Leadership Foundation of the ABFM, I have developed a keen interest in innovative practice redesign. Following the lead of pioneers in Direct Patient Care like Erika Bliss, M.D. at Qliance in Seattle, I plan to spend the second half of my career as a DPC physician. I believe the DPC model is the key to coverage with access in the future; however, in the short-term, my goal is to find innovative ways for family physicians, nurse practitioners and physician assistants to collaboratively solve the access problem. I believe the healthcare system can only deliver on the "triple aim" with DPC at its base. The disruptive innovation can only occur by first proving to employers that they will see reduced cost, better outcomes and better employee satisfaction by purchasing primary care directly. As larger and larger companies see these benefits, DPC and the evolving insurance products that "wrap around" it can disrupt the system.
