Hello again,

Thank you very much for your feedback, it helps more than you know. If you don't mind I have a few more questions off this "cultural assessment guide". Again I really appreciate your help! If you prefer not to answer any of the questions, I'll understand.

What is your name?
Primary language:
Place of birth:
Educational level or specialized training:
To which ethnic group do you belong?
Who is the spokesperson for your family?
Can you describe some of your family's dietary habits and your personal preferences?
Are there any foods forbidden from you diet for religious or cultural reasons?
Who/what is your primary source of information about your health?
On whom do you rely on health care services or healing?
Are there religious restrictions that your caregiver should know?
What are the beliefs and views on marriage? Are there arranged marriages?
What are the thoughts on infidelity? Divorce?
If caring for a family member in a nursing home and is going to die, is there someone to prepare the body?
Is there a particular process for the funeral?

I think that these are the majority of the questions to be asked. When I give the report I'm supposed to provide handouts for the class. Is there anything that you can think of that would benefit? If not its okay.
