My library years ago came very close to sending out a survey on computer equipment that asked:

How large is your floppy dick?

This embarassing typo was found on the very last proofing before it went out in campus mail.

The library vendor where I used to work, in the early days of the Web, included a numeric URL in a newsletter article. It was supposed to go to some library-esque site. But thanks to a typo it went to Hot Asian Babes. The staffer who discovered (and as it turned out, made) the error started screaming uncontrollably when she typed it in. It was a good day.
