Introduction	My proudest achievements as a medical informatics physician have been my contributions to the HL7 Reference Information Model and the development of the templated HL7 Continuity of Care CDA Document. In addition, I am very proud of the number of physicians and other clinicians who first learned their EHR patient care skills using the Horizon Physician Portal.
