This explains, in part, how the discrepancy between the number of active physicians and the amount of incentive money proposed will be met.&lt;/span&gt;&lt;/span&gt;&lt;/span&gt;&lt;/div&gt;&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;2. The  American Recovery and Reinvestment Act of 2009&lt;span class="Apple-style-span" style="font-weight: bold; "&gt;&lt;span class="Apple-style-span"  style=" ;font-size:16px;"&gt;&lt;span class="Apple-style-span"  style=" font-weight: normal; font-size:13px;"&gt;&lt;span class="Apple-style-span"  style=" ;font-size:16px;"&gt; &lt;/span&gt;&lt;span class="Apple-style-span"  style="font-size:small;"&gt;calls for the "meaningful use" of Electronic Health Record software&lt;/span&gt;&lt;span class="Apple-style-span"  style=" ;font-size:16px;"&gt;. &lt;span class="Apple-style-span"  style=" ;font-size:13px;"&gt; What does "meaningful use" mean?  It probable includes e-prescribing, integration with the laboratory, and measures of Quality of Care.  Towards these ends, ICANotes is implementing e-prescribing imminently and is looking for a partner that will cooperate in the integration of laboratory and icanotes databases. This two way integration will include the sending of lab requests from ICANotes to the laboratory and the importation of laboratory results into the patient record.&lt;/span&gt;&lt;/span&gt;&lt;/span&gt;&lt;/span&gt;&lt;/span&gt;&lt;/div&gt;&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;&lt;span class="Apple-style-span" style="font-weight: bold; "&gt;&lt;span class="Apple-style-span"  style=" ;font-size:16px;"&gt;&lt;span class="Apple-style-span"  style=" font-weight: normal; font-size:13px;"&gt;&lt;span class="Apple-style-span"  style=" ;font-size:16px;"&gt;&lt;span class="Apple-style-span"  style=" ;font-size:13px;"&gt;Quality of care measurement will involve the incorporation into ICANotes of outcome measurement tools, which is planned. &lt;/span&gt;&lt;/span&gt;&lt;/span&gt;&lt;/span&gt;&lt;/span&gt;&lt;/div&gt;&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;3.  To get full incentive benefits a physician has to be using EHR in 2011, as we know.  How long should the user have been using in order to be considered a "meaningful user."?  Good question.  One panelist thought EHR should have been in use for 6-12 months.&lt;/div&gt;&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;4, The requirement that  every patient be in an Electronic Health Record has been  proposed for 2014,&lt;/div&gt;&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;5,  While CCHIT has not yet been officially designated the certifying body for electronic health records, there is no other agency ns sight who could perform the function and scant time to develop another such agency. &lt;/div&gt;&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;Ira Morganstern, M.D.&lt;/div&gt;&lt;/span&gt;&lt;/span&gt;&lt;/div&gt;&lt;div&gt;&lt;span class="Apple-style-span"   style="border-collapse: collapse;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;Incentive payments begin in 2011.&lt;o:p&gt;&lt;/o:p&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(E) PAYMENT YEAR DEFINED.— &lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="mso-spacerun:yes"&gt; &lt;/span&gt;‘‘(i) IN GENERAL.—For purposes of this subsection, &lt;/p&gt;  &lt;p class="MsoNormal"&gt;the term &lt;span style="color:red"&gt;‘payment year’ means a year beginning with &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:red"&gt;2011.&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;It would appear that in addition to the other incentives, the professional (or employer or facility) will receive an additional amount of money equal to 75% of the allowed Medicare charges for the payment year.&lt;span style="mso-spacerun:yes"&gt;  &lt;/span&gt;Therefore, the more billed to Medicare, the higher the incentives&lt;/b&gt;.&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;(a) INCENTIVE PAYMENTS.—Section 1848 of the Social Security &lt;/p&gt;  &lt;p class="MsoNormal"&gt;(42 U.S.C. 1395w–4) is amended by adding at the end the &lt;/p&gt;  &lt;p class="MsoNormal"&gt;wing new subsection: &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(o) INCENTIVES FOR ADOPTION AND MEANINGFUL USE OF CER-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;ED EHR TECHNOLOGY.— INCENTIVE PAYMENTS.— &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(1) I&lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(A) IN GENERAL.— &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(i) IN GENERAL.—Subject to the succeeding sub-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;paragraphs of this paragraph, with respect to covered &lt;/p&gt;  &lt;p class="MsoNormal"&gt;professional services furnished by an eligible profes-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;sional during a payment year (as defined in subpara-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;graph (E)), if the eligible professional is a meaningful &lt;/p&gt;  &lt;p class="MsoNormal"&gt;EHR user (as determined under paragraph (2)) for &lt;/p&gt;  &lt;p class="MsoNormal"&gt;the EHR reporting period with respect to such year, &lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;in addition to the amount otherwise paid&lt;/span&gt; under this &lt;o:p&gt;&lt;/o:p&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;part, &lt;span style="color:red"&gt;there also shall be paid&lt;/span&gt;&lt;/b&gt; to the eligible professional &lt;/p&gt;  &lt;p class="MsoNormal"&gt;(or to an employer or facility in the cases described &lt;/p&gt;  &lt;p class="MsoNormal"&gt;in clause (A) of section 1842(b)(6)), from the Federal &lt;/p&gt;  &lt;p class="MsoNormal"&gt;Supplementary Medical Insurance Trust Fund estab-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;lished under section 1841 &lt;b style="mso-bidi-font-weight: normal"&gt;&lt;span style="color:red"&gt;an amount equal to 75 per-&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;cent of &lt;/span&gt;&lt;/b&gt;the Secretary’s estimate (based on claims sub-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;mitted not later than 2 months after the end of the &lt;/p&gt;  &lt;p class="MsoNormal"&gt;payment year) of &lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;the allowed charges under this part &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;for all such covered professional services furnished by &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;the eligible professional during such year&lt;/span&gt;&lt;/b&gt;&lt;span style="color:red"&gt;.&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:red"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;Incentives stop after 2016&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:red"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(ii) NO INCENTIVE PAYMENTS WITH RESPECT TO &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;YEARS AFTER 2016.—&lt;/span&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;No incentive payments may be made under this subsection with respect to a year after 2016&lt;/span&gt;&lt;span style="color:black"&gt;.&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;Incentives cannot start after 20014&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(v) NO INCENTIVE PAYMENT IF FIRST ADOPTING AFTER 2014.—&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;If the first payment year for an eligible &lt;/span&gt;&lt;/b&gt;&lt;span style="color:black"&gt;&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;professional is after 2014 then the applicable amount &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;specified in this subparagraph for such professional &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;for such year and any subsequent year shall be $0.&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;Regarding the amount of the incentive, notice below that there is a bonus for starting in 2011 or 2012. If one of those years are the first payment years, then the payment is $18,000 instead of $15,000.&lt;span style="mso-spacerun:yes"&gt;  &lt;/span&gt;Apparently there is a desire to incentivize early adopters of EHR.&lt;span style="mso-spacerun:yes"&gt;  &lt;/span&gt;&lt;o:p&gt;&lt;/o:p&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="mso-spacerun:yes"&gt; &lt;/span&gt;Notice also, above, that incentives cannot start after 2014. So, the only allowed start years are 11, 12, 13, and 14 and there are bonuses for 11 and 12. &lt;o:p&gt;&lt;/o:p&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;Also, please note that if the earliest one can receive an incentive is 2010 and the incentive lasts five years and there are no payments after 2016, then in order to receive the complete incentive the first payment year can be no later than 2012. Those starting their incentives in 13 or 14 will not get the full amount!&lt;o:p&gt;&lt;/o:p&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;MOUNT.—Subject to clauses (iii) through (v), &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(ii) A&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;the applicable amount specified in this subparagraph &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;for an eligible professional is as follows: &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;‘‘(I) &lt;/span&gt;&lt;span style="color:red"&gt;For the first payment year for such profes-&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;sional, $15,000 (or, if the first payment year for &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;such eligible professional is 2011 or 2012, $18,000). &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(II) For the second payment year for such &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;professional, $12,000. &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(III) For the third payment year for such &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;professional, $8,000. &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(IV) For the fourth payment year for such &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;professional, $4,000. &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(V) For the fifth payment year for such profes-&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;sional, $2,000. &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(VI) For any succeeding payment year for &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;such professional, $0.&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;What does the following “phase down” paragraph mean?&lt;span style="mso-spacerun:yes"&gt;  &lt;/span&gt;Does it mean that if you start payment after 2013 you start payments at $12,000 and end payments in 2016?&lt;span style="mso-spacerun:yes"&gt;  &lt;/span&gt;I think so. I guess that is because the hospital is getting its own incentive.&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(C) NON-APPLICATION TO HOSPITAL-BASED ELIGIBLE &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;PROFESSIONALS.—&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;&lt;span style="mso-spacerun:yes"&gt; &lt;/span&gt;&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(i) IN GENERAL.—No incentive payment may be &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;made under this paragraph in the case of a hospital- &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;based eligible professional.&lt;/span&gt; &lt;span style="color:black"&gt;‘‘(ii) HOSPITAL-BASEDELIGIBLEPROFESSIONAL.—For &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;purposes of clause (i), the term&lt;b style="mso-bidi-font-weight:normal"&gt; &lt;/b&gt;&lt;/span&gt;&lt;b style="mso-bidi-font-weight: normal"&gt;&lt;span style="color:red"&gt;‘hospital-based eligible &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;professional’ means&lt;/span&gt;&lt;span style="color:black"&gt;, &lt;/span&gt;&lt;/b&gt;&lt;span style="color:black"&gt;with respect to covered profes-&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;sional services furnished by an eligible professional &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;during the EHR reporting period for a payment year, &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;an eligible professional, such as a&lt;/span&gt;&lt;span style="color:black"&gt; &lt;/span&gt;&lt;span style="color:red"&gt;pathologist, anesthe-&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;siologist, or emergency physician,&lt;/span&gt;&lt;span style="color:black"&gt; &lt;/span&gt;&lt;/b&gt;&lt;span style="color:black"&gt;who furnishes &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;substantially all of such services in a hospital setting &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;(whether inpatient or outpatient) and through the use &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;of the facilities and equipment, including qualified elec-&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;tronic health records, of the hospital. The determina-&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;tion of whether an eligible professional is a hospital- &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;based eligible professional shall be made on the basis &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;of the site of service&lt;/span&gt;&lt;span style="color:red"&gt; &lt;/span&gt;&lt;span style="color:black"&gt;(as defined by the Secretary) &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;and without regard to any employment or billing &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;arrangement between the eligible professional and any &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;other provider.&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;All the above is the carrot. Here is the stick. After 2014 non users of EHR will have their Medicare payments reduce by 1 or 2 or possibly as much as 5%.&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:black"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(7) INCENTIVESFORMEANINGFULUSEOFCERTIFIEDEHR &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;TECHNOLOGY.— &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;DJUSTMENT.— &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(A) A&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(i) INGENERAL.—Subject to subparagraphs (B) &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;and (D), with respect to covered professional services &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;furnished by an eligible professional &lt;/span&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;during 2015&lt;/span&gt;&lt;/b&gt;&lt;span style="color:black"&gt; or &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;any subsequent payment year,&lt;/span&gt;&lt;/b&gt;&lt;span style="color:red"&gt; &lt;b style="mso-bidi-font-weight:normal"&gt;if the eligible profes-&lt;o:p&gt;&lt;/o:p&gt;&lt;/b&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;sional is not a meaningful EHR user&lt;/span&gt;&lt;/b&gt;&lt;span style="color:black"&gt; (as determined &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;under subsection (o)(2)) for an EHR reporting period &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;for the year, the fee schedule amount for such services &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;furnished by such professional during the year &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;(including the fee schedule amount for purposes of &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;determining a payment based on such amount) shall &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;be equal to the applicable percent of the fee schedule &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;amount that would otherwise apply to such services &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;under this subsection (determined after application of &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;paragraph (3) but without regard to this paragraph). &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;PPLICABLEPERCENT.—Subject to clause (iii), &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(ii) A&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;for purposes of clause (i), the term ‘applicable percent’ &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;means— &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(I) for &lt;/span&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="color:red"&gt;2015, 99 percent&lt;/span&gt;&lt;/b&gt;&lt;span style="color:black"&gt; (or, in the case of &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;an eligible professional who was subject to the &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;application of the payment adjustment under sec-&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;tion 1848(a)(5) for 2014, 98 percent); &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="color:black"&gt;‘‘(II) &lt;/span&gt;&lt;span style="color:red"&gt;for 2016, 98 percent&lt;/span&gt;&lt;span style="color:black"&gt;;  &lt;p class="MsoNormal" style="margin-left:.5in"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal" style="margin-left:.5in"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;INCENTIVEPAYMENTS.— &lt;/p&gt;  &lt;p class="MsoNormal"&gt;(1) I&lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(A) INGENERAL.— &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(i) INGENERAL.—Subject to the succeeding sub-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;paragraphs of this paragraph, with respect to covered &lt;/p&gt;  &lt;p class="MsoNormal"&gt;professional services furnished by an eligible profes-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;sional during a payment year (as defined in subpara-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;graph (E)), if the eligible professional is a meaningful &lt;/p&gt;  &lt;p class="MsoNormal"&gt;EHR user (as determined under paragraph (2)) for &lt;/p&gt;  &lt;p class="MsoNormal"&gt;the EHR reporting period with respect to such year, &lt;/p&gt;  &lt;p class="MsoNormal"&gt;in addition to the amount otherwise paid under this &lt;/p&gt;  &lt;p class="MsoNormal"&gt;part, there also shall be paid to the eligible professional &lt;/p&gt;  &lt;p class="MsoNormal"&gt;(or to an employer or facility in the cases described &lt;/p&gt;  &lt;p class="MsoNormal"&gt;in clause (A) of section 1842(b)(6)), from the Federal &lt;/p&gt;  &lt;p class="MsoNormal"&gt;Supplementary Medical Insurance Trust Fund estab-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;lished under section 1841 an amount equal to 75 per-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;cent of the Secretary’s estimate (based on claims sub-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;mitted not later than 2 months after the end of the &lt;/p&gt;  &lt;p class="MsoNormal"&gt;payment year) of the allowed charges under this part &lt;/p&gt;  &lt;p class="MsoNormal"&gt;for all such covered professional services furnished by &lt;/p&gt;  &lt;p class="MsoNormal"&gt;the eligible professional during such year. &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(ii) NO INCENTIVEPAYMENTSWITHRESPECTTO &lt;/p&gt;  &lt;p class="MsoNormal"&gt;YEARSAFTER2016.—No incentive payments may be&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;MOUNT.—Subject to clauses (iii) through (v), &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(ii) A&lt;/p&gt;  &lt;p class="MsoNormal"&gt;the applicable amount specified in this subparagraph &lt;/p&gt;  &lt;p class="MsoNormal"&gt;for an eligible professional is as follows: &lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="font-size:16.0pt;"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="font-size:16.0pt;"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="font-size:16.0pt;"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="font-size:16.0pt;"&gt;Q: How much is the incentive and how is it distributed?&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="font-size:16.0pt;"&gt;A:&lt;span style="mso-spacerun:yes"&gt;  &lt;/span&gt;$15 k the first year, $12 the second, $8 the third, $4 the fourth, $2 the fifth, for a total of $41,000.&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="font-size:16.0pt;"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(I) For the first payment year for such profes-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;sional, $15,000 (or, if the first payment year for &lt;/p&gt;  &lt;p class="MsoNormal"&gt;such eligible professional is 2011 or 2012, $18,000). &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(II) For the second payment year for such &lt;/p&gt;  &lt;p class="MsoNormal"&gt;professional, $12,000. &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(III) For the third payment year for such &lt;/p&gt;  &lt;p class="MsoNormal"&gt;professional, $8,000. &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(IV) For the fourth payment year for such &lt;/p&gt;  &lt;p class="MsoNormal"&gt;professional, $4,000. &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(V) For the fifth payment year for such profes-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;sional, $2,000. &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(VI) For any succeeding payment year for &lt;/p&gt;  &lt;p class="MsoNormal"&gt;such professional, $0.&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="font-size:16.0pt;"&gt;Q:&lt;span style="mso-spacerun:yes"&gt;  &lt;/span&gt;Any other incentives?&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="font-size:16.0pt;"&gt;A: Yes, an extra 10% for physicians working in “health professional shortage areas.”&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;is 2013. &lt;/p&gt;  &lt;p class="MsoNormal"&gt;INCREASEFORCERTAINELIGIBLEPROFES-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(iv) I&lt;/p&gt;  &lt;p class="MsoNormal"&gt;SIONALS.—In the case of an eligible professional who &lt;/p&gt;  &lt;p class="MsoNormal"&gt;predominantly furnishes services under this part in &lt;/p&gt;  &lt;p class="MsoNormal"&gt;an area that is designated by the Secretary (under &lt;/p&gt;  &lt;p class="MsoNormal"&gt;section 332(a)(1)(A) of the Public Health Service Act) &lt;/p&gt;  &lt;p class="MsoNormal"&gt;as a health professional shortage area, the amount &lt;/p&gt;  &lt;p class="MsoNormal"&gt;that would otherwise apply for a payment year for &lt;/p&gt;  &lt;p class="MsoNormal"&gt;such professional under subclauses (I) through (V) of &lt;/p&gt;  &lt;p class="MsoNormal"&gt;clause (ii) shall be increased by 10 percent. In imple-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="font-size:16.0pt;"&gt;Q: Is there a cut off for these incentives?&lt;br /&gt;A: Yes, incentives end in 2014. There are no incentives for eligible professionals who apply in 2014 or subsequently&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(v) NO INCENTIVEPAYMENTIFFIRSTADOPTING &lt;/p&gt;  &lt;p class="MsoNormal"&gt;AFTER2014.—If the first payment year for an eligible &lt;/p&gt;  &lt;p class="MsoNormal"&gt;professional is after 2014 then the applicable amount &lt;/p&gt;  &lt;p class="MsoNormal"&gt;specified in this subparagraph for such professional &lt;/p&gt;  &lt;p class="MsoNormal"&gt;for such year and any subsequent year shall be $0.&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;E) P&lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(i) I&lt;/p&gt;  &lt;p class="MsoNormal"&gt;NGENERAL.—For purposes of this subsection, &lt;/p&gt;  &lt;p class="MsoNormal"&gt;the term ‘payment year’ means a year beginning with &lt;/p&gt;  &lt;p class="MsoNormal"&gt;2011. &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(ii) FIRST, &lt;span style="mso-spacerun:yes"&gt; &lt;/span&gt;SECOND,&lt;span style="mso-spacerun:yes"&gt;  &lt;/span&gt;ETC.&lt;span style="mso-spacerun:yes"&gt;  &lt;/span&gt;PAYMENTYEAR.—The &lt;/p&gt;  &lt;p class="MsoNormal"&gt;term ‘first payment year’ means, with respect to cov-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;ered professional services furnished by an eligible &lt;/p&gt;  &lt;p class="MsoNormal"&gt;professional, the first year for which an incentive pay-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;ment is made for such services under this subsection. &lt;/p&gt;  &lt;p class="MsoNormal"&gt;The terms ‘second payment year’, ‘third payment year’, &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘fourth payment year’, and ‘fifth payment year’ mean, &lt;/p&gt;  &lt;p class="MsoNormal"&gt;with respect to covered professional services furnished &lt;/p&gt;  &lt;p class="MsoNormal"&gt;by such eligible professional, each successive year &lt;/p&gt;  &lt;p class="MsoNormal"&gt;immediately following the first payment year for such &lt;/p&gt;  &lt;p class="MsoNormal"&gt;professional.&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="font-size:16.0pt;"&gt;Q: What does “meaningful use of Certified HIT mean?&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="font-size:16.0pt;"&gt;A:&lt;span style="mso-spacerun:yes"&gt;  &lt;/span&gt;It means what the Secretary defines it to mean, but means that the EHR is certified and that e-prescribing is included.&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(i) MEANINGFULUSEOFCERTIFIEDEHRTECH-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;NOLOGY.—The eligible professional demonstrates to the &lt;/p&gt;  &lt;p class="MsoNormal"&gt;satisfaction of the Secretary, in accordance with &lt;/p&gt;  &lt;p class="MsoNormal"&gt;subparagraph (C)(i), that during such period the profes-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;sional is using certified EHR technology in a meaning-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;ful manner, which shall include the use of electronic &lt;/p&gt;  &lt;p class="MsoNormal"&gt;prescribing as determined to be appropriate by the &lt;/p&gt;  &lt;p class="MsoNormal"&gt;Secretary.&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="font-size:16.0pt;"&gt;Q: How do you prove you are using Certified EHR in a meaningful way?&lt;br /&gt;A:&lt;span style="mso-spacerun:yes"&gt;  &lt;/span&gt;You can “attest” to it.&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(C) DEMONSTRATIONOFMEANINGFULUSEOFCERTIFIED &lt;/p&gt;  &lt;p class="MsoNormal"&gt;TECHNOLOGYANDINFORMATIONEXCHANGE.— &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(i) INGENERAL.—A professional may satisfy the &lt;/p&gt;  &lt;p class="MsoNormal"&gt;demonstration requirement of clauses (i) and (ii) of &lt;/p&gt;  &lt;p class="MsoNormal"&gt;subparagraph (A) through means specified by the Sec-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;retary, which may include— &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(I) an attestation; &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(II) the submission of claims with appropriate &lt;/p&gt;  &lt;p class="MsoNormal"&gt;coding (such as a code indicating that a patient &lt;/p&gt;  &lt;p class="MsoNormal"&gt;encounter was documented using certified EHR &lt;/p&gt;  &lt;p class="MsoNormal"&gt;technology); &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(III) a survey response; &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(IV) reporting under subparagraph (A)(iii); &lt;/p&gt;  &lt;p class="MsoNormal"&gt;And&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span class="Apple-style-span"  style=" font-weight: bold;font-size:48px;"&gt;&lt;br /&gt;&lt;/span&gt;&lt;/p&gt; INCENTIVES FOR HOSPITALS. &lt;/p&gt;  &lt;p class="MsoNormal"&gt;(a) INCENTIVE PAYMENT.— &lt;/p&gt;  &lt;p class="MsoNormal"&gt;(1) I&lt;/p&gt;  &lt;p class="MsoNormal"&gt;NGENERAL.—Section 1886 of the Social Security Act &lt;/p&gt;  &lt;p class="MsoNormal"&gt;(42 U.S.C. 1395ww) is amended by adding at the end the &lt;/p&gt;  &lt;p class="MsoNormal"&gt;following new subsection: &lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;b style="mso-bidi-font-weight:normal"&gt;&lt;span style="font-size:16.0pt;"&gt;Q: How much is the incentive to a hospital?&lt;br /&gt;A: $2 million from this bill and $2 million from the &lt;st1:place st="on"&gt;&lt;st1:placename st="on"&gt;Federal&lt;/st1:placename&gt; &lt;st1:placetype st="on"&gt;Hospital&lt;/st1:placetype&gt;&lt;/st1:place&gt; Insurance Trust Fund for a total of $4 million. &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;span style="font-size:16.0pt;"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(n) INCENTIVESFOR ADOPTIONAND MEANINGFUL USEOF CER-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;TIFIED EHR TECHNOLOGY.— &lt;/p&gt;  &lt;p class="MsoNormal"&gt;NGENERAL.—Subject to the succeeding provisions of &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(1) I&lt;/p&gt;  &lt;p class="MsoNormal"&gt;this subsection, with respect to inpatient hospital services fur-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;nished by an eligible hospital during a payment year (as defined &lt;/p&gt;  &lt;p class="MsoNormal"&gt;in paragraph (2)(G)), if the eligible hospital is a meaningful &lt;/p&gt;  &lt;p class="MsoNormal"&gt;EHR user (as determined under paragraph (3)) for the EHR &lt;/p&gt;  &lt;p class="MsoNormal"&gt;reporting period with respect to such year, in addition to the &lt;/p&gt;  &lt;p class="MsoNormal"&gt;amount otherwise paid under this section, there also shall &lt;/p&gt;  &lt;p class="MsoNormal"&gt;be paid to the eligible hospital, from the &lt;st1:place st="on"&gt;&lt;st1:placename st="on"&gt;Federal&lt;/st1:placename&gt; &lt;st1:placetype st="on"&gt;Hospital&lt;/st1:placetype&gt;&lt;/st1:place&gt; &lt;/p&gt;  &lt;p class="MsoNormal"&gt;Insurance Trust Fund established under section 1817, an &lt;/p&gt;  &lt;p class="MsoNormal"&gt;amount equal to the applicable amount specified in paragraph &lt;/p&gt;  &lt;p class="MsoNormal"&gt;(2)(A) for the hospital for such payment year.&lt;/p&gt;  &lt;p class="MsoNormal"&gt;&lt;o:p&gt; &lt;/o:p&gt;&lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘(2) P&lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(A) INGENERAL.—Subject to the succeeding subpara-&lt;/p&gt;  &lt;p class="MsoNormal"&gt;graphs of this paragraph, the applicable amount specified &lt;/p&gt;  &lt;p class="MsoNormal"&gt;in this subparagraph for an eligible hospital for a payment &lt;/p&gt;  &lt;p class="MsoNormal"&gt;year is equal to the product of the following: &lt;/p&gt;  &lt;p class="MsoNormal"&gt;NITIALAMOUNT.—The sum of— &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(i) I&lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(I) the base amount specified in subparagraph &lt;/p&gt;  &lt;p class="MsoNormal"&gt;(B); plus &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(II) the discharge related amount specified &lt;/p&gt;  &lt;p class="MsoNormal"&gt;in subparagraph (C) for a 12-month period selected &lt;/p&gt;  &lt;p class="MsoNormal"&gt;by the Secretary with respect to such payment &lt;/p&gt;  &lt;p class="MsoNormal"&gt;year. &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(ii) M&lt;/p&gt;  &lt;p class="MsoNormal"&gt;EDICARESHARE.—The Medicare share as &lt;/p&gt;  &lt;p class="MsoNormal"&gt;specified in subparagraph (D) for the eligible hospital &lt;/p&gt;  &lt;p class="MsoNormal"&gt;for a period selected by the Secretary with respect &lt;/p&gt;  &lt;p class="MsoNormal"&gt;to such payment year. &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(iii) TRANSITIONFACTOR.—The transition factor &lt;/p&gt;  &lt;p class="MsoNormal"&gt;specified in subparagraph (E) for the eligible hospital &lt;/p&gt;  &lt;p class="MsoNormal"&gt;for the payment year. &lt;/p&gt;  &lt;p class="MsoNormal"&gt;‘‘(B) BASEAMOUNT.—The base amount specified in this &lt;/p&gt; Here's how to do that:&lt;br /&gt;&lt;br /&gt;&lt;span class="Apple-style-span" style="font-family: arial; font-size: 13px; "&gt;&lt;br /&gt;1.  Go to the reports menu at the top.&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;2. Select Billing Report&lt;/div&gt;&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;3.  Clear all fields except for the Patient Name field.&lt;/div&gt;&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;4. Make a report, which will be a list of all the notes for that patient.&lt;/div&gt;&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;5.  Use the button at the top the report field, "Print These patient representative calls to arrange treatment, a new chart can be created and the Telephone Intake Form, which is located by scrolling down the Patient Demographic Page, can be filled out.  By filling out this form a new chart is started, valuable information is input,  and many fields in the Patient Demographic section will already be filled out when the patient arrives for his or her This is useful, for example, when the user wants to paste the finished note into an email or a word document or into another program, such as an enterprise wide medical record database.&lt;div&gt;&lt;br /&gt;&lt;/div&gt;&lt;div&gt;There are now two buttons on the Preview screen. If it is set as "yes" the box on Progress Note two will be marked to "Include Medical History in note;  x yes." If left checked as "yes", the Medical History will appear in the progress note and in all subsequent progress notes.&lt;br /&gt;&lt;br /&gt;But, how ever the preference is set, it can be overrode while a progress note is being made. That is, the "Include Medical History in note" on PN 2 can be checked or unchecked by the user at any time.
