The presentation will include tips on how to become visible within the organization so your name will be considered for a leadership opportunity.</p><p><strong>About the Speaker</strong></p><p>Rod completed a 20-year career of active duty service in 1997 as a Commander in the Supply Corps of the Navy. Notable assignments include the Freight Terminal Director, Naval Supply Depot, Subic Bay, Philippines during the withdrawal of the Navy after the Mount Pinatubo volcano eruption in 1991 and Project Manager for the Navy's implementation of the Worldwide Port System, an automated ocean cargo documentation system developed by the Army and implemented at all military cargo handling port facilities. Rod was the first Supply Corps officer to earn qualification in all three primary Supply Corps warfare designations of submarines, air warfare and surface warfare in addition to qualifying for the Basic Parachutist and Naval Parachutist warfare devices.</p><p>Rod received his B.A. from Iowa State and his MBA from Penn State. He is certified as a Project Management Professional (PMP) with the Project Management Institute; certified as a Quality Engineer (CQE), Quality Manager (CMQ/OE), Qualify Auditor (CQA) and Software Quality Engineer (CSQE) with the American Society for Quality; Certified Production & Inventory Management (CPIM) and Certified Information Resources Management (CIRM) with the American Production & Inventory Control Society; Certified Professional Logistician (CPL) with the Society of Logistics Engineers; Certified in Transportation & Logistics (CTL) with the American Society of Transportation and Logistics; and Certified Tester (CSTE) with the Quality Assurance Institute. However, the data analysis often leaves the stories respondents tell out of the final report and lost to decision makers. Open-ended questions are often left unanalyzed or fed back without a supporting structure from which fact-based decisions can be drawn. As a result, the voice of the respondent is often diminished or distorted. </p><p>Some of the more common barriers to the effective use of open-ended questions are: poorly written questions, ineffective data analysis and unproductive presentation of results. When used properly, open-ended data can provide a story that enriches the quantitative results of a survey or questionnaire, while measuring up to the same rigorous methodological standards as the quantitative data. </p><p>This interactive session provides an introduction into the methodology for analyzing open-ended questionnaire data. Bob holds a doctorate in Organizational Psychology from Saint Louis University.</p><p><strong>Second Presentation</strong><br /><strong style="color: rgb(0, 0, 153);">Mistake Proofing - Not Just for Manufacturing</strong><br />Presented by Sam Hoskins, Mistake-Proofing<br /></p>Mistake-Proofing is a primary Lean tool as developed by Shigeo Shingo and Toyota. They found that human error was the most prevalent cause of defects. In any aspect of any process, the essential component is people, and even highly skilled people make mistakes. These errors are the root causes that develop into larger consequences.<br /><br />Mistake-Proofing seeks to prevent those basic errors from happening in the first place. Therefore, the first step to effective Mistake-Proofing is to accept that human errors do occur. This presentation will give many real examples, including heathcare, of Mistake-Proofing devices and how they were developed.<br /><br />Using a simple root-cause and brainstorming process, a device or method can be easily developed which will prevent that specific error from ever happening again. This presentation is an overview of human errors, the conditions that can set us up for failure, and finally an examination of some specific tools needed to quickly develop effective Mistake-Proofing devices, permanently eliminating the possibility of that error happening again.<br /><br />There is ample opportunity to use Mistake-Proofing in healthcare. An oft-quoted statistic from the Institute of Medicine says there are up to 98,000 patient deaths per year resulting from human errors.<br /><br />As the Lean tools continue to migrate from manufacturing into healthcare, Mistake-Proofing will be shown to be invaluable. The primary advantage of using Mistake-Proofing is realized by keeping a narrow focus on the problem you are trying to prevent. Working in small teams, and using a reliable method, look at your latest error and brainstorm a low-cost/no-cost solution to keep it from reoccurring.<br /><br />The best ideas are often simple ones. He spent 12 years as a manufacturing quality engineer and quality manager for an international commercial explosives manufacturer. His projects included ergonomic and safety improvements to the workplace, machine reliability, various cost reductions, and, of course, Mistake-Proofing.<br /><br />Sam authored the Mistake-Proofing portion of the Ensign-Bickford Company’s application for the 2002 Shingo Prize for Excellence in Manufacturing, which they won. He has led dozens of successful M-P events, which resulted in large gains in safety, quality, and productivity.<br /><br />He is an experienced instructor in the workplace and has also taught several aircraft technology courses at Southern Illinois University. He holds a Bachelor’s degree in Mechanical Engineering Technology, a Master’s in Manufacturing Systems, is a certified Incident Commander and is an ASQ-certified Six Sigma Black Belt.<br /><br />About a year and a half ago, Sam started MistakeProofing.Net to provide consulting and hands-on training to manufacturing and healthcare organizations. Also, the presentation will demonstrate the RED method to assist with some of the existing drawbacks of failure analysis. RED is a risk assessment method that can be applied as early as the conceptual design phase with minimal functional information needed from the product undergoing analysis. In order to mitigate risk and failures RED links failures to product function. The failure linkage also exists between product components so that as the design matures, so can the FMEA analysis. These linkages provide designers the ability to use catalogued historical failures as to prevent failures in new designs or redesigns. RED reuses failure information via matrix manipulations which calculate preliminary risk likelihood and consequence for specific areas of a product. This information is used to contribute significantly to initializing an FMEA. The information automatically pinpoints and ranks areas of concern in the product so that engineers can focus the significant portion of their efforts on risk mitigation.</p><p>Dr. Grantham Lough joined the Interdisciplinary Engineering Department of the University of Missouri - Rolla as an Assistant Professor in January of 2006. At UMR she develops and applies risk assessment methodologies to product design principles. These methodologies are aimed at educating novice engineers about critical issues in the design process. Before joining the UMR faculty, Dr. Grantham Lough served as a research scientist for 21st Century Systems where she currently holds a part time position. As part of 21st Century Systems she has added risk assessment techniques to their existing defense software products. Also, she was involved with projects to identify both hardware and software failures in mechatronic systems.</p><p><a name="0907second"></a><strong>Second Presentation:<br /></strong><span style="FONT-WEIGHT: bold; COLOR: rgb(51,0,153)">Dialogic approaches to organizational learning and effectiveness</span><br /><em>Presented by Ray Luechtefeld, Ph.D.</em></p><div style="FLOAT: left; MARGIN-RIGHT: 10px"><img alt="Dr. Ray Luechtefeld" src="http://www.asqstlouis.org/images/Ray_Luechtefeld.jpg" /></div><p>Ray will present a common focus on the centrality of communication and dialogue to organization to increase team effectiveness. Fairly simple interventions in a conversation can significantly improve team performance. An experiment involving more than one hundred teams working in a problem-solving simulation showed that exposure to these interventions improved the quantitative measure of team performance. Improved team performance is the result of an improved ability to constructively handle conflict. Teams exposed to interventions are able to share information more effectively and come up with win-win solutions more readily than teams not exposed to the interventions.</p><p>Dr. Luechtefeld is an Assistant Professor at the Department of Engineering Management and Systems Engineering in the University of Missouri - Rolla. He received his B.S. degree in Electrical Engineering from the University of Missouri - Rolla, his MBA from the University of Minnesota, and his Ph.D. from Boston College. Dr. Luechtefeld has nine years of industrial experience as a Development Engineer with IBM. The third tuesday in November falls during Thanksgiving this year and the third tuesday in December falls less than a week before Christmas.
